Is the supply wiring properly fused?

Does the wiring conform to local and national electrical codes?

Is a safety disconnect switch supplied?

Record inspection and maintenance performed

Check for moisture or dirt accumulations on parts

Describe the source of moisture or dirt, if found

Ensure the fan is turned off and locked out before starting lubrication

Verify that the lubricant is a high quality lithium complex grease conforming to NLGI Grade 2

Are unusual conditions present (temperatures below 32°F or above 200°F, moisture, or contaminants)?

Add lubricant with the shaft rotating

Continue adding lubricant until clean grease is seen purging from the bearing

Is the bearing visible to observe purged grease?

Enter the number of shots of grease applied if bearing is not visible

Ensure the unit is attached to the designed system

Verify system static pressure is adequate

Is the wheel rotation in the same direction as the airflow at the outlet?

Does the wheel rotation conform to the rotation decal affixed to the unit?

Was the wheel rotation correct?

Run each fan to operating temperature

Bump each fan if not run to full operating temperature

Have operating conditions of the fan changed?

If yes, describe the changes in operating conditions

Consult manufacturer for safety confirmation on new operating conditions
