Inspect Valve Body Seat(s) for nicks and scratches and other signs of excessive wear

Inspect Valve Stem & Seat(s) rubber or PTFE seat(s) for signs of cracking, tearing, checking, or excessive wear

Inspect stem finish for galling, nicks, or scratches

Inspect Split Bushing for signs of cracks, nicks, scratches or excessive wear on inside diameter

Inspect O-ring(s) for cuts, abrasions, flat spots, or other damage

Upload photos of any damage found

Sign off on the valve inspection

Warning: Disconnect and remove actuator completely from valve before proceeding

Bushing and packing both need replacement?

If only one end fails, both ends of the actuator should be serviced

Upload a photo of the removed actuator

Any worn or damaged components found?

Replace all worn or damaged components as required

Stem and o-ring sprayed with Sanitary Lubricant?

All valve bodies mated properly before securing with clamps?

All clamps tightened securely to assure proper alignment?

Warning: Ensure the valve stem is secured before proceeding

Nylon Locking Pin located in the middle of the stem thread?

Old nylon pin punched out with an 1/8 inch or smaller diameter punch?

New nylon locking pin inserted?

Sign off on the actuator stem replacement

Warning: Over lubrication is not a remedy for actuator failure

Oil injected into the unthreaded thru hole in each end of the actuator

Actuator rolled on a flat surface for uniform coverage

Sign off on the actuator lubrication
