General Work Permit

This procedure ensures that all non-routine work activities are formally reviewed and authorized through a structured general work permit process, verifying that potential hazards have been identified and controlled, appropriate safety measures are in place, and all personnel involved are informed of relevant risks and precautions before work commences, supporting a safe and compliant work environment in accordance with applicable safety regulations and organizational standards.

    EMF / Medical Implant Check

Does any person carrying out the work have a pacemaker, ICD, or other implanted/body-worn medical device that may be affected by electromagnetic fields (EMF)?

If Yes – STOP. The potential EMF exposure must be assessed before work begins.

    Asbestos Check

Will the work involve drilling, cutting, breaking, fixing into, removing or otherwise disturbing the fabric of the building or existing insulation/materials?

If Yes – the Asbestos Register / relevant asbestos information must be checked before work begins.

    2. Pre-Work Safety Checks

The following checks must be completed before the permit can be authorised.

Has a suitable risk assessment / RAMS been reviewed for the work?

Have the people carrying out the work been confirmed as competent and authorised?

Has the work area been inspected before starting?

Has the scope of work been clearly understood by everyone involved?

Is the correct equipment available and suitable for the task?

Has equipment been inspected and confirmed safe for use?

Are suitable access and egress arrangements available?

Is the work area adequately lit?

Is suitable housekeeping in place and are trip/slip hazards controlled?

Have nearby employees and other persons who may be affected by the work been considered?

Have barriers, cones, warning signs or an exclusion zone been provided where required?

Are emergency escape routes and fire exits clear?

    3. Plant / Equipment Isolation

Does the work require any electrical, pneumatic, hydraulic, mechanical, steam, gas or other energy source to be isolated?

    If Yes:

Have all relevant energy sources been identified?

Has the plant/equipment been safely isolated?

Has stored or residual energy been released or secured?

Has Lock Out / Tag Out or the site's isolation procedure been applied where required?

Has isolation been verified before work begins?

    Isolation reference / details:

    4. Asbestos

Before any work which could disturb the fabric of the building, insulation, panels, ceilings, walls, flooring, pipework or other existing materials, asbestos information must be reviewed. HSE requires information on the location and condition of asbestos-containing materials to be made available to people who may disturb them during maintenance or other work.

Could the work disturb the building fabric or existing materials?

    If Yes:

Has the current Asbestos Register / survey information been checked for the work location?

Has the exact work area been checked against the asbestos information?

Is the area/material confirmed as suitable for the proposed work?

Has the person carrying out the work been made aware of any known or suspected asbestos-containing materials nearby?

Is a refurbishment/demolition or further intrusive asbestos survey required before the work can proceed?

    STOP WORK

If asbestos-containing material is known or suspected and the proposed work could disturb it, work must not commence until the appropriate asbestos assessment, survey and controls are in place.

If any unknown or suspicious material is discovered during the work:

STOP WORK IMMEDIATELY – do not disturb the material further and report it to the responsible person.

    Asbestos Register / Survey reference:

    5. Electromagnetic Fields (EMF)

Certain electrical equipment, high-current cables, induction equipment, welding equipment and other machinery may generate electromagnetic fields. These may present an additional risk to people with pacemakers, ICDs and certain other implanted or body-worn medical devices.

Will equipment capable of generating significant EMF be used or worked near?

    If Yes:

Has the potential EMF hazard been assessed?

Has consideration been given to persons with implanted or body-worn medical devices?

Are any persons who may be affected by an implanted/body-worn medical device involved in or near the work?

    If Yes:

STOP – DO NOT COMMENCE WORK

A suitable individual EMF assessment must be completed and any required exclusion distance or additional controls established before work begins. HSE identifies workers with active implanted medical devices, including cardiac pacemakers, as potentially requiring additional consideration around EMF sources.

Details of EMF controls / exclusion zone:

    6. Additional Hazards

Identify any hazards associated with the work:

    Additional hazards / precautions required:

    7. PPE Requirements
