This form must be completed by the designated operator before the start of the shift.

Leaks / Goteos

Tire Damage or Wear / Neumáticos dañados o desgastados
3-ENGINE COMPARTMENT / COMPARTIMIENTO DEL MOTOR
1.-Oil level / Nivel de aceite
2.-Battery Condition / Estado de la batería
3.-Check that the radiator is clean and in good condition. / Verificar que el radiador esté limpio y en buen estado.
4.-Air Filter / Filtro de Aire
5.-Fork Condition / Estado de las horquillas (tenedores)
6.- Mast Operation / Funcionamiento de la torre
7.- Check that the fork locking pins work properly. / Verificar que los pasadores de bloqueo de las horquillas funcionen correctamente.
8.-Check that the data plate is present and readable. / Verificar que la placa de datos esté presente y sea legible.
OPERATIONAL INSPECTION / INSPECCIÓN OPERATIVA

2-Indicator and Warning Lights / Luces indicadoras y de advertencia
3-Parking Brake / Freno de estacionamiento
4-Lifting Control / Control de levantamiento

6-Accelerator / Acelerador
7-Steering / Volante
8-Brakes / Frenos
9-Backup Alarm / Alarma de reversa
10.-Check that the lights work properly. / Verificar que las luces funcionen correctamente.
11.-Horn / Claxon Check that the horn works properly. / Verificar que el claxon funcione correctamente.
12.-Oil Spots on Floor / Manchas de aceite en el piso
13.-Personal Protective Equipment / Equipo de protección personal
CAUTION: This is not a complete list of all items which may require attention. Operators are responsible for ensuring that the lift truck is in proper working condition in accordance with the manufacturer’s specifications. If you need training and/or a certification to use this or any other equipment, please talk to your Shift Supervisor. DO NOT PERFORM REPLACEMENTS AND/OR MAINTENANCE OPERATIONS WITHOUT THE ADEQUATE PERSONAL PROTECTIVE EQUIPMENT.
