Signing this document acknowledges the Pretium Employee is fully LOTO trained, has been given the proper Equipment to lock out all Machinery/Equipment (locks, tags, hasps). Also to understand the SEVERITY of locking out the machines/equipment as well as the Corrective Actions that can be enforced if the employee is caught not locking out machines/equipment.

ENG: I Agree the training that was provided by the Assistant Manager of Maintenance and the Manager of Safety, on the IMPORTANCE of using LOTO. I agree that I am now responsible for making sure that all machines/equipment are properly locked out when there is any servicing and or maintenance done to the machine/equipment. De-energizing machines/equipment is IMPORTANT to my safety as well as my co-workers. ESP: Acepto la capacitación impartida por el Subgerente de Mantenimiento y el Gerente de Seguridad sobre la IMPORTANCIA de usar el sistema LOTO. Acepto que ahora soy responsable de asegurarme de que todas las máquinas/equipos estén correctamente bloqueados al realizar cualquier servicio o mantenimiento. Desconectar las máquinas/equipos es IMPORTANTE para mi seguridad y la de mis compañeros.
ENG: Corrective Actions of NOT Locking out. / ESP: Acciones Correctivas de NO Bloqueo.
ENG: I agree that I have been told about the 3 warnings I will be given if I am caught without following the proper procedure(s) of LOTO. ESP: Estoy de acuerdo en que me han informado sobre las 3 advertencias que recibiré si no sigo los procedimientos adecuados de LOTO.
