The 'Take 5' procedure is a comprehensive prestart check and hazard identification process designed to ensure safety before commencing a task. It includes verifying permits, identifying hazards, implementing control measures, ensuring proper training and equipment, and confirming isolation and safety readiness.
Prestart Check
Is there a procedure or SOP for this job? If yes, check the procedure or SOP for all of the potential hazards.
Are you going to excavate, drill or penetrate any surface? If yes, attach an Excavation and Penetration permit.
Are you performing servicing or maintenance activities in which a zero energy state is not feasible? If yes, a Partial Energy Permit must be completed in addition to a Risk Assessment.
Will this work generate heat or sparks? If yes, attach a Hot Work Permit unless the work is a production task undertaken in a designated production hot work area or maintenance work in a maintenance hot work area.
Are you going to enter a Confined Space? If yes, attach Confined Space Permit, risk assessment and rescue plan.
Will this task require you to work at heights? If yes, attach Working at Heights Permit.
Will you need to use a crane to lift loads without approved lifting attachments or using rated lifting points? If yes, a Rigger or Dogman must assess the lift.

Can you now do this job safely? If NO contact your Supervisor for Instruction
