This procedure documents issues using a Problem, Cause, and Solution (PCS) format to ensure clear root-cause analysis and effective resolution tracking. It guides users to describe the problem, identify its root cause through analysis (such as the 5 Whys), and record the implemented solution with supporting images.

    Problem:

    Describe the Problem that occurred. Add support images of the problem to better describe the problem if possible.

    Problem Image:

    Add or drag pictures

    Cause:

    What was the root cause of the Problem? Perform a 5 why as needed to determine the best root problem.

    Solution:

    What was the Solution to this issue. Add images to support what the fix was if possible.
