Question 1: Describe the purpose of the LOTO procedure.

• That all energy sources have been de-energised and equipment has been established as safe to work on. This includes all electrical, mechanical, thermal, hydraulic and chemical energy sources. • That precautions have been established to ensure that an energy source cannot be accidently re-energised.

• That all energy sources have been energised and equipment has been established as unsafe to work on. This includes all electrical, mechanical, thermal, hydraulic and chemical energy sources. • That precautions have been established to ensure that an energy source cannot be accidently re-energised.

Question 2: Define 'Competent' in the context of LOTO.

• A person who has acquired knowledge by training and experience, and has the necessary skill and where required qualification to perform the task, and in performing that task acts diligently and competently.

• All persons that understand and comply with the requirements of the Permit to Work procedure. Removal of a lock or tag belonging to another person is accepted.

Question 3: What is a Danger Tag and when is it used?

• This tag allows for faulty, unsafe or permanently disabled equipment to be taken out of service to prevent further use or damage to the equipment. Any item of equipment or plant marked with an “Out of Service” tag SHALL NOT be operated or used except for the direct purpose of repair by an authorised person.

• This tag is used to identify the isolation points or apparatus, equipment, or lines, which shall remain de-energised and isolated for work to be safely performed. This tag can only be used with the ‘Red Lock’ and must only be removed by the person who placed the lock & tag. For electrical equipment, this tag is normally attached to the disconnect switch, breaker, racking mechanism and/or control handle. For mechanical equipment, the tag is to be attached to the controlling device (handle/operating lever) of the energy isolation. Unauthorised removal of a ‘DANGER DO NOT OPERATE’ tag and lock or operation of any equipment secured by the tag and lock will result in Disciplinary Action.

Question 4: Describe the steps for placing Personal Locks and Danger Tags.

• • Isolations SHALL be performed by – Any persons with knowledge of the location. • Isolations shall be proven i.e. valve confirmed shut, or supplies confirmed off, and secured by an Operations Isolation Lock(s) (Blue) and Danger Tag/s . • When application of the isolation has been completed and they have been informed it is safe to do so the work / test party technicians SHALL then place their Personal Locks (Red) and Danger Tags to secure the isolation either by placing on each of the Isolation Point(s) Hasp(s) or securing the ‘lock out’ box containing the isolation control (keys, fuses, etc.).

• • Isolations SHALL be performed by – Suitably qualified and trained persons with knowledge of the location and deemed competent under this procedure. • Isolations shall be proven i.e. valve confirmed shut, or supplies confirmed off, and secured by an Operations Isolation Lock(s) (Blue) and Danger Tag/s . • When application of the isolation has been completed and they have been informed it is safe to do so the work / test party technicians SHALL then place their Personal Locks (Red) and Danger Tags to secure the isolation either by placing on each of the Isolation Point(s) Hasp(s) or securing the ‘lock out’ box containing the isolation control (keys, fuses, etc.).

Question 5: Raising a Lockout Box

• The Lockout Box is used in conjunction with a JSA / SWMS and provides individual or group protection from injury caused by the inadvertent operation of equipment. Control keys or devices securing the points of isolation are placed in the box with indication of the locations. The lock out box may contain other documentation relating to the isolation, including copies of SCAP. The work party will secure the Lock Box to prevent access to or interference with the isolation points, by each placing their personal locks to secure the lock box. Only personnel working under a specific JSA / SWMS who have their Personal Lock and Personal Danger Tag attached to the Lockout Box are protected by the Lockout Procedures. The following steps are to be followed when raising a Lockout Box:

• The Team Leader or HV Operator will be responsible for creating the required isolation instruction for each JSA / SWMS. (The isolations SHALL be checked as correct by a second Service Technician or HV Operator as appropriate). Where the isolation is complex this may be further reviewed by system engineer or HVAP

• The Team Leader or HV Operator will obtain an unused Lockout Box, including the Lockout Box Lock (Yellow), the required amount of Operations Isolation Locks (Blue) and also the required amount of Danger Tags for the Isolations to be secured.

• The Team Leader or HV Operator using the relevant JSA / SWMS isolation instructions, SHALL correctly identify each of the apparatus Isolations, and instruct or carry out the Isolation

• The Team Leader or HV Operator will then attach a hasp, and secure it with an Operations Isolation Lock (Blue) and Danger Tag on each of the Isolation Points to secure the isolations. This action will be recorded by signing that the Isolations has been completed & checked on the JSA / SWMS isolation instruction. Where the point of isolation is a fuse, link, or removable spool. The fuse, link, or spool may be removed and secured in the lock out box. The item must be labelled indicating from where it was removed and the point of isolation SHALL be tagged i.e. the fuse carrier will have suitably danger tag applied.

• The Team Leader or HV Operator shall place all of the Operations Isolation Lock Keys and isolating items inside the Lockout Box. (Where fuses or isolating items are too large, they SHALL be chained and padlocked to a suitable structure on the site, and the key secured in the Lockout box).

• When all Isolations have been performed and initialled on the JSA / SWMS, or switching instruction. The Team Leader or HV Operator will then secure the Lockout Box with a Lockout Box Lock (Yellow) and retains the Lockout Box Key (This key may be attached to the EAP/VAP/SFT or work documents to demonstrate work party has control of the isolation)

• The Work party team leader, or in the case of HV work the electrical access permit (EAP/VAP/SFT) RIC/TIC SHALL ensure that all personnel involved in the work activity, including themselves, place their Personal Lock(s), and Personal Danger Tag(s) on the Lockout Box Hasp before any work commences.
