    Date and time

    Name of requester

    Please add telephone number or email for quicker response

    MACHINE LOCATION

    Please select which line has the fault

    Enter the machine name

    Please describe machine name or position on the line eg. EC2 Zero labour , FM3000 , Sub assembly c/v

    Select the type of fault

    Describe the fault

    Attach pictures / files / video's etc

    The more information Randek have the quicker we can resolve the fault

    Add or drag pictures

    List actions taken by Bellway to fix the fault

    RECORD OF ACTION AFTER REPORT RAISED

    SUPPORT TICKET NUMBER

    Once support email has been sent a ticket number will be issued. Please use this ticket number in all communication with Randek UK or Randek Sweden
