General Requirements
Site Administration
Submit/revise Facility and Tank UST forms
Submit/revise Cert. of Compliance Form for Install
Submit/revise Financial Responsibility paperwork
Submit written contract between owner and operator
Obtain valid UST operating permit
Submit/revise facility plot plan showing UST systems
Submit/revise routine Monitoring Procedure
Submit/revise Release Response Plan
Document alarms, releases, and maintenance records
Document tightness tests comply with state regs
Provide annual calibration/testing records for leak detection equipment in required format
Provide monitoring system annual certification tag
Provide certificate of training for monitoring system tester
W/in 1,000 ft. of well:

Provide enhanced leak detection
System Maintenance and Operation
Remove product from sumps, dispenser pans
Find and repair leak(s)
Provide required cathodic protection system inspection/documentation
Provide required inspections for lined tanks
Secure monitoring wells
Repair monitoring system
Cease placing panel in “silence” mode
Provide dispenser pans.
Provide/repair dispenser pan monitoring
Repair tank system
Replace turbine sump gasket(s)
Provide required secondary containment testing
