Monthly TGW Sorter Preventive Maintenance

LOTO Shutdown Procedure:

• 1. Ensure all affected areas and employees are aware of the pending shut-down.

• 2. Verify that it is safe to shut down the equipment.

• 3. Perform normal shutdown procedures for the specified piece of equipment.

• 4. Isolate the power source for the piece of equipment and shut down that source of power.

• i. Immediately affix his/her LOTO lock to the device, so that it ensures the lock is in a position that will not allow the device to be turned back on without removing the lock and/or is.

• ii. Enter required information on tag and apply to device in a conspicuous location

• 1. If the placement of the tag would compromise safety (e.g. obstructing the view of an indicator light) then place the tag as close to the device’s on/off mechanism as possible.

• 5. Release stored energy.

• i. In cases of stored mechanical energy, vent valves, spring releases, blocking devices, or equipment repositions must be utilized.

• ii. In cases of stored electrical energy, approved grounding wands or discharge devices must be used.

• iii. If the possibility of the re-accumulation of stored energy exists, verification of safety must continually be done until the maintenance being performed is complete.

• 6. Perform verification of shutdown.

Verification of shutdown:

• 1. Attempt to turn on the piece of equipment that has been shut down.

• i. The authorized employee must personally test the equipment to ensure it will not turn on – use normal startup procedures designed for the piece of equipment be maintained.

• ii. This is to ensure the piece of equipment WILL NOT turn on.

• 2. Test for zero-energy state (if electrical)

• i. The authorized employee must personally test potential electrical energy sources using appropriate meters and/or testers.

• ii. In the event the authorized employee is not qualified to test an energy source, a qualified tester may be used.

• 1. Ensure any qualified tester used is noted on the remarks section of the tag

• iii. Ensure all testing equipment is inspected for proper use before and after all tests for a zero-energy state.

Release from LOTO:

• 1. Verify that it is safe to reenergize and ensure all work for which the LOTO was applied has been completed and it is safe to reenergize the equipment.

• 2. Clear all equipment, tools and personnel from the work area to ensure that personnel and equipment is at a safe distance.

• 3. Remove all isolating and grounding devices.

• 4. Replace safety guards (as required). Ensure that any safety guards taken off prior to LOTO are reinstalled.

• 5. Remove LOTO devices and reset the energy-isolating device, and return the machine/equipment that received maintenance to service.

• 6. Notify all affected personnel that the equipment is back in service.

• i. In the event that the equipment was not fixed and requires follow-on maintenance, notify affected personnel and place an ‘out of service tag’ on the piece of equipment.
