To be used in conjunction with Confined Space Permit. Add additional procedure for each entrant.

    Entry Log

    The Safety Observer (SO) is to record the following details for each person entering the confined space:

    Name of Entrant

    Date and Time of Entry

    Entrant Sign In

    Time of Exit

    Entrant Sign Out

    SO's Initials
