1. Task Understanding
Do I fully understand the job, scope, and steps involved?
Am I trained and competent to perform this task?
Hazard Identification
What could go wrong? (e.g., falls, moving plant, electricity, hazardous substances)
Are there any changes to the environment, weather, or site conditions?
Does the work require Hot Works?
if the answer is "oneOf [Yes]"
Does the work require Lock Out Tag Out?
if the answer is "oneOf [Yes]"
Can be effectively isolated using no more than two isolators and has two or less energy sources
if the answer is "oneOf [Yes]"
Simple Isolation
if the answer is "oneOf [No]"
Complex isolation
Does the work require entry into a confined space?
if the answer is "oneOf [Yes]"
Risk Assessment
Is this a non-routine, complex, or high-risk task (e.g., working at heights, confined spaces, hot works, lifting operations, complex LOTO)?

Must complete risk assessment of task
Add or drag pictures
Are existing controls (SWMS, permits, PPE, supervision) enough?
If not sure, STOP and complete a hard copy (paper-based) risk assessment.
Control Measures
Have the hazards been eliminated, isolated, or controlled and are exclusion zones needed and are in place?
Do I have access to the correct PPE, tools, and equipment?
Is the work area safe and set up correctly?
Final Check
Am I fit for work (alert, not fatigued, no impairments)?

Inform your supervisor
Have I communicated with my team and supervisor?
Do I know how to stop work and escalate if conditions change?
